If you are a first time patient, you will need to fill out the Personal Authorization Forms before coming to your first visit with us. In an effort to save you time prior to your appointment, we offer downloadable forms for you to fill out in the comfort of your own home. Simply print the designated forms, and bring them completed with you to the office.

Financial Policy
​
At New Life Clinic we participate with the following insurance providers: Wellmark, Avera Health Plans, Sanford, Dakotacare, Medicare, South Dakota Medicaid, Tricare (United Health Care), and Medica. We accept Visa and MasterCard. Co-pays are required at time of service.
